  • Anal Fistula: A disease you never want to have

An anal fistula is the disease of the anus characterized by a swollen node around the anal region. It usually develops as an abnormal communication between the epithelial surface of the anal canal and perianal skin. It’s a very common problem and occurs mostly in people with previous anal infection. Fistula in the anus resulting from an anal infection or abscess originates from the anal glands that start to drain making anal fistula even worse. What are the Various Types of Anal Fistula? Man suffering from anal fistula, Credits: pixabay There are four types of Anal Fistula which are described as follows: Inter sphincteric Fistula: This form of fistula involves the sphincter muscles and generally occurs between the anus’ external and inner sphincter muscles. Then it gradually passes through the internal sphincter muscles close towards the anus. Tran sphincteric Fistula: This form of fistula originates between the external and inner sphincter muscles in the region behind the anal area. It flows across the outer sphincter and the anus away from the ischiorectal fossa to the perineum. It is sometimes called a horseshoe fistula, since it often takes a U-turn and may form many outside openings. Supra Sphincteric Fistula: The gross anal fistula is about 5 percent. The abscess is caused by the supra-levator between the inner and outer sphincter muscles. This stretches between the puborectal muscles and the levator ani downwards. It is an inch or so away from the anus. Extra Sphincteric Fistula: It occurs when a foreign body enters the rectum, or because of the Crohn’s disease and rectal carcinoma, as the name suggests. It starts from the rectum or sigmoid colon and devolves into the anus’ tissue. What are the Symptoms of Anal Fistulas? Pain, redness and itching due to anal fistula, Credits: pixabay Anal fistulae may irritate the skin, cause itching, swelling, redness, and pain in the anus and around the anal region. The area may feel sore and tender Pus discharge and blood can be seen with the movements of the bowel Treatment options for Anal fistula Anal fistula treatment is carried out by two methods: Traditional Surgical Method Laser Surgery These are further explained as follows: Traditional Methods of treatment Representation of anal fistula, Credits: pixabay Treatment of anal fistula through traditional methods is performed very cautiously in order to reduce the risk of bowel restriction in individual. There are various treatment options available which are listed below: Fistulotomy This is done in the fistulous tract and the surrounding tissue by making an incision. It is performed by making an incision on the fistulous tract and the surrounding tissue. The surgeon will then scrape off all the pus and its contents which are then flushed out. After flushing out the sides are stitched to lay open the fistula. Advancement Rectal Flap In patients with poor muscles the operation is performed. During this operation, the surgeon cuts the rectal wall like a flap in order to reach, scrape and scratch the pus from the fistula’s internal opening. Seton Placement A silk thread or a rubber band is attached to facilitate the drainage in this procedure. Fibrin Glue or Collagen plug Fibrin glue is a process where the surgeon will use Fibrin glue or collagen plug to monitor tract contents after they are draining. The internal opening is stitched and closed. Laser Surgery for Anal Fistula Laser operation is one of the specialized care options for Anal Fistula patients. Laser energy passes near the anal fistula, which shrinks and kills the epithelium of the fistula. It is very brief and patients will restart work within 24 hours. Anal fistulas can be a most embarrassing situation for the person who is suffering from the same. But with proper treatment, hygiene and lifestyle management the disease can be managed.

  • Why Do Clots Form and How Dangerous are They?

We have blood flowing in the veins and arteries all throughout our body blood clot in the brain. It is typically continuously flowing and is circulated in the body by the heart. However, the moment a person is injured or starts bleeding, it is important for the blood to start forming a clot in order to stop the bleeding. How are Clots Formed? Blood cells consist of red blood cells, white blood cells, and platelets. When a blood vessel gets injured, it will constrict in order to try and limit blood flow. In response to injury and inflammation, the platelet cells will get activated. In doing so, the platelets will release more chemicals that will attract other platelets to migrate to the site of injury on the blood vessel. After migration, the platelets stick to each other and to the blood vessels with the help of a protein, von Willebrand factor (VWF). Now there are some additional proteins in your blood known as clotting factors. They cause a rapid chain reaction that triggers the production of fibrin. Fibrin is a strand-like substance that surrounds the platelet plug and forms a mesh-like net around the platelet plug, keeping it in a fixed place. This mesh-like structure is known as a blood clot. Thus, one could say that platelets are essentially blood cells with structural function to obstruct unnecessary blood flow to stop your body from bleeding out and the proteins in your blood help form fibrin, which will hold these platelets in place, keeping it stable and intact. The clot and meshy networks will continue to strengthen but will eventually dissolve when the injured vessels heal. When does it get Dangerous? Internal blood clot, Credit: Wikimedia commons Typically, clots will stop your blood from escaping the blood vessels when there is an injury. However, in many cases, the process of clotting can be unnecessary and far more dangerous than helpful to your body. This is when the blood flowing in your vessels gets triggered and starts to abnormally clot inside your blood vessels. When this happens, the normal and necessary blood flow throughout the body gets obstructed and as a result, the body cells will not receive enough oxygen from the RBCs and start to get damaged. For example, a blood clot in the brain could seriously damage normal brain function and cause harmful effects. Blood clot in the brain symptoms include: Weakness Problems with vision and speech Seizures Dizziness Headache Another dangerous case is when the arteries of the heart get blocked by a clot. Clots in the brain and heart are usually formed when the cholesterol plaques formed in the blood vessels break open and trigger the clotting process. Sometimes, clots can also form when blood does not flow properly. Lastly, head concussion or some form of internal injury could also trigger clot formation. One way to try to remove blood clot in the brain or any part of the body naturally is to consume foods and substances that act as a natural blood thinner that breaks down clots. They include: Ginger Cinnamon Turmeric Ginger and turmeric, Credit: Wikimedia commons Way to Prevent Dangerous Clot Formation They include: Avoiding eating junk food: This is because junk food contains extremely high levels of cholesterol which can cause plaques on the walls of the arteries which can burst and start clot formation. Maintaining a good diet: As mentioned above, there are food substances that act as natural blood thinners. In addition to that, deficiency in proteins can also cause clot formation protein S and protein C play a major role in controlling the clotting process. Exercise: Regular exercise will help regulate your blood flow and pressure and also prevent obesity. Moreover, aerobic exercise has also been known to reverse blood clot formation. Takeaway Clotting of blood may be an essential mechanism of the human body but sometimes, it can be fatal and could result in death. Thus, It is very important for your doctor to identify an internal blood clot. In fact, some internal blood clots originating from the arms and legs can travel to your lungs. This is known as pulmonary embolism and a person suffering from it could be in great danger.
