What Is Follicular Lymphoma? Follicular lymphoma is a slow-progression type of non-Hodgkin lymphoma. Lymphocytes are white blood cells that are a very important part of the immune system and helps us fight off infections. B and T cell are the two types of lymphocytes of our immune system. Lymphoma is a type of blood cancer that occurs when these lymphocytes grow in big numbers. Lymphomas are categorized into Hodgkin lymphomas and Non-Hodgkin lymphomas (NHL). It is one of the most common types of non-Hodgkin lymphoma. It is known as ‘follicular’ because of development of B cells in the lymph nodes in clumps. Rituximab has completely changed the outlook of this disease and has significantly improved the overall survival of patients in the last two decades. The average survival is more than 20 years if treatment is taken. How Common Is Follicular Lymphoma? This type of lymphoma is is more commonly seen in older patients over the age of 60. Its occurrence rate in India is at 2.9 per 1 lac for men and 1.5 per lac for women. Studies have shown that its occurrence rates are higher in the Western countries as compared to Asian or African countries. Follicular lymphoma in children is very rare. Variants of Follicular Lymphoma The two types of Follicular Lymphoma are : 1. Duodenal-type follicular lymphoma This follicular lymphoma type is usually found in the small intestine. It grows slowly and can be diagnosed in its early stages. It responds to treatment very well and doesn’t spread to other parts of the body. Duodenal-type follicular lymphoma is very unlikely to turn into an aggressive form of lymphoma. 2. Pediatric-type follicular lymphoma As the name depicts, this is a type of lymphoma which affects children and is proven to be very rare in its occurrence. It is usually found in the lymph nodes of head or neck. Similar to duodenal-type follicular lymphoma, this also can be diagnosed in its early stages and responds very well to treatment. Studies have shown very low relapse rates after successful treatment. What Are The Symptoms Of Follicular Lymphoma? Its symptoms are not sudden and they present over a period of time. Some people may not develop symptoms at all and its diagnosis may be incidental when tests are performed for some other medical issue. The most common symptom of follicular lymphoma is swelling of lymph nodes. These enlarged lymph nodes are usually found in the neck area, arm pits and groin area. Over a period of time, the B cell lymphocytes start growing in huge numbers and form a build up and leads to enlargement of lymph nodes. Causes of Follicular Lymphoma The cause of follicular lymphoma largely remains unknown just like other forms of cancer. It is not believed to be genetic and cannot be passed down in families. Environmental factors may contribute to development and aggravate this form of cancer. It is linked to exposure to deadly substances such as pesticides, dyes, industrial solvents, benzene etc. but the available data is inconsistent and requires further validation. It has also been linked to smoking. Certain lifestyle factors such as obesity, sedentary lifestyle and unhealthy food are believed to be associated with an increased the risk of follicular lymphoma. In most of the cases, development of follicular lymphoma cannot be attributed to any cause or risk. At the same time, lots of people with risk factors never go on to develop this condition. Risk Factors for Follicular Lymphoma Some factors that are believed to increase the risk of follicular lymphoma are listed below : Medications that suppress your immune system Immunosuppressants are prescribed to patients for a variety of diseases, mostly for auto immune conditions and when a patient undergoes transplant. This, in turn compromises the patient’s auto immune system and its ability to fight infections. Infection with certain viruses and bacteria Epstein-Barr and HIV are two viruses that are believed to increase the risk of non-Hodgkin’s lymphoma. Influenza vaccination and heart diseases are to also linked with an increased risk but this requires further validation. Chemicals Insecticides, pesticides, industrial solvents, benzene etc. may increase your risk of follicular lymphoma. Old age It is known to occur at all ages, but people over the age of 60 contribute to a significant portion of diagnosed cases. Hence, it is believed that the risk of follicular lymphoma increases with age. Grading Of Follicular Lymphoma Follicular Lymphoma is divided into four categories on the basis of number of centroblast (large cells) that are observed. It is common in this type of cancer to get diagnosed at advanced stages when the bone marrow is also infiltered. At the same time, if diagnosed in early stages, it can be cured completely in one third cases with no recurrence. Stage I: Only one lymph node area or one organ is involved Stage II – More than two lymph node areas are involved on the same side of diaphragm Stage III – When cancer is found on both sides of the diaphragm Stage IV – This stage indicates widespread disease involving multiple regions or organs. When the lymphoma reaches liver or bone marrow, it is always classified as stage IV. At the time of stage assignment, A or B letter is also included to denote whether the patient has B symptoms (fever, weight loss and night sweats). Diagnosis and Staging Physical Examination A physical examination includes feeling lymph nodes in the most common regions where they occur to check if they are enlarged. Neck, armpits and groin regions are usually checked for swollen lymph nodes. The doctor may also check for any signs of swelling in other regions such as throat, spleen and liver, which is usually accompanied by a condition of lymphatic system. Biopsy Biopsy is the most important part of diagnosis. A sample is taken from suspected affected lymph node to check for lymphoma cells. Blood Test The blood investigations include a complete blood count, kidney, and liver function tests, and signs for any viral infections such as Hepatitis B and C, HIV. Enzyme lactate dehydrogenase (LDH) levels are also checked as part of the investigation since one fourth of the patients with follicular lymphoma have elevated levels of LDH. What Is The Best Treatment of Follicular Lymphoma The treatment course will be decided by your doctor on the basis of classification of disease stage. Stage I and II are considered as early stages whereas Stage III and IV are considered as advanced stages of lymphoma. The treatment will be influenced by patient’s age, health condition, location of the lymphoma, size of the enlarged nymph nodes and result of investigations. Treatment of Early-Stage Follicular Lymphoma Only 1 in every 10 people diagnosed are at an early stage of follicular lymphoma. At this stage, radiotherapy is usually the most preferred treatment. If it is located in only one part of organ of the body, then radiation therapy is considered to be the most suitable and most effective, often curing the disease completely. A recent study suggested that a combination of radiation therapy and systemic or chemoimmunotherapy is more effective than radiation therapy alone. In a scenario where radiotherapy is not a suitable treatment choice, observation is considered as a reasonable alternative or monoclonal antibody treatment such as Rituximab is advised. The treatment at this stage has been very successful in recent years. The treatment aims to keep the lymphoma under control with a few side effects. Most people feel better with treatment and sometimes relapse is seen. If the lymphoma is causing problems, chemotherapy combined with antibody therapy is done. The most common treatments used are Bendamustine, also used in the treatment of chronic lymphocytic leukemia. This is used after other treatments have not worked R-CVP – Rituximab + cyclophosphamide, vincristine, and prednisolone R-CHOP – Rituximab + cyclophosphamide, doxorubicin, vincristine and prednisolon Studies have shown that R-CVP and R-CHOP are both very effective in treatment of follicular lymphoma. R-CVP is the preferred choice for younger patients. The common drugs used in antibody therapy are rituximab and obinutuzumab. Maintenance treatment of antibody therapy is offered to patients who respond well to chemo-immunotherapy treatment. This is done to slow down the progression of diseases and delay instances of recurrence. There are several antibody therapies currently being tested for follicular lymphoma. Stem Cell Transplant In relapsing forms of follicular lymphoma, a stem cell transplant may be considered. In this procedure, healthy stem cells are transplanted into the bone marrow or blood. The tubing, valve, and diaphragm create the vacuum suction pressure that is required to drain the breast. The volume of milk expressed by the breast pump is determined by the vacuum suction pressure and the cycle frequency generated by the pump and the amount of breast fullness. Poor suction pressure can limit milk expression leading to early termination of breastfeeding. Extreme suction pressure can cause discomfort and breast injury and affect long-term breastfeeding success. Despite manufacturer recommendations, breastfeeding expert advice and differences in pump design, some women interchange kits between different manufacturers of breast pumps. Currently, there are no studies that evaluate whether interchanging the brand of kit impacts breast pump performance. The design of the pump prevents contamination from one user to another. These pumps tend to be more expensive than personal-use electric pumps, precluding ownership by most lactating mothers. ADJUSTABLE SUCTION PRESSURES AND CYCLE SPEED Nearly all pumps, both electric and manual, allow the mother to alter vacuum pressures. However, many pumps will automatically increase the cycle duration as the vacuum pressure increases, causing the cycles to be longer duration and slower in frequency. Conversely the same pumps decrease cycle duration with lower vacuum pressure, causing the cycles to become shorter and more frequent. Immunohistochemistry revealed that the dominant dermal cells responding to the larva were activated T-helper cells which, in turn, probably stimulate plasma cell production and B-cells to produce antibody. The activated fibroblasts produce collagen that may be important in containment of the larva within its fibrous, subcutaneous nodule. The first instar larvae produced the greatest host reaction and allowed the earliest detection of infestation by ELISA. However, the initial immune response against antigens produced by second and third instar larvae was depressed during the course of an infestation, but started to increase as soon as the larvae left their host. This suggests that immunosuppression may be a phenomenon. Portchinsky (1916) recorded many cases of human infestation with W. mugnca from Russia, with approximately 80% in children of 10 years or less and approximately 70% affecting the ears. Most other cases affected the head area, either eyes, mouth or nose. Human infestations due to W. magnca are still recorded, from the ear (El-Begenny, 1989), eye nose or mouth. Rhinosporidiosis is a chronic, granulomatous infection caused by Rhinosporidium seeberi. It is characterised by the development of granular, polypoidal masses primarily affecting the mucous membranes of the nasal cavity and nasopharynx. The disease is more prevalent in South India and Sri Lanka. Cutaneous, laryngotracheal, genital and bony dissemination are rare, and can mimic soft tissue sarcoma. Following the onset of nasal lesions, it usually takes many years for systemic dissemination to occur. Very rarely rhinosporidiosis involves larynx and trachea with other nasophryngeal lesions. Rhinosporidiosis is a chronic disease which affects the mucous membrane of nose and larynx. however, tracheal dissemination is rare. If the larynx and trachea get involved, then there is a potential risk of aspiring the contents. Locally this can spread by spilling of the spores from pharynx into larynx during a surgery etc. Rhinosporidiosis affects mucous lining of nose and pharynx. Rarely spreads to trachea and if spreads then it is very dangerous. Nasopharyngeal lesions are seen predominantly in males, while ocular infection is more prevalent among females. Infection produces granulomatous inflammation of the affected tissues. The infection usually begin as papules but transform into large polypoidal masses which bleed on touching. They resemble strawberry with red-pink color and white specs, specially in the nose. Masses are usually unilateral and pedunculated, and rarely exteriorise. Other mucosal sites are involved infrequently, including the palpebral conjunctiva, oropharynx, asopharynx, maxillary antrum, larynx, external ear canal, parotid duct and genitalia. Oral and oropharyngeal lesions may interfere with breathing and/or eating. Systemic dissemination to the trachea, lung, urethra, liver, genitalia, lacrimal sac and bone is very rare. Dissemination can be fatal, and occurs more commonly in association with immunodeficiency disorders (such as acquired immunodeficiency syndrome). Infection can spread via body fluids such as ascites and blood. If the airways is blocked. Rather, a CT scan or computed tomography should be done to evaluate the extent of the lesions in larynx and trachea. Patients affected by it can complain of voice being hoarse over a period of time. Also, the sputum can be filled with blood on coughing. The throat could be affected the most by this. If the nose will be examined then red-pink strawberry like irregular lesions with nodules could have white specs on them and may extend further into the pharynx(naso and laryngopharynx). Fibre-optic laryngoscopic examination revealed multiple sessile lesions occluding the tracheal lumen, more on the right lateral wall than the left. Opacities of the soft tissue in nose and pharynx, larynx, trachea can be diagnosed with the help of computed tomography (CT). Routine haematological investigations, chest radiography and fundoscopy can also be conducted. Diode laser excision of lesions was performed under direct laryngoscopy and rigid bronchoscopy guidance. Histopathological examination of the excised specimens revealed hyperplastic epithelium with globular cysts of various shapes, representing sporangia at various stages of development Post-operatively, the patients can be commenced on dapsone 100 mg/day, which he was advised to continue for two years. Blood enters the cavernous sinuses from the ophthalmic veins, the superficial middle cerebral veins, inferior cerebral veins and the sphenoparietal sinuses, as well as from the sphenoid sinuses via communicating veins in the intervening bone. The cavernous sinuses drain via emissary veins into the pterygoid venous plexus, and via the inferior and the superior petrosal sinuses draining into the internal jugular vein and the sigmoid sinus respectively. The cavernous sinuses and their connections are devoid of valves, consequently bidirectional spread of infection and thrombi can occur throughout this network. Infection of the thrombus within the cavernous sinuses cannot be proven in the vast majority of antemortem cases of septic CST, but has been demonstrated in postmortem studies. Infection may spread within veins from the primary source to the cavernous sinuses as a contiguous phlebitis/ thrombophlebitis which are trapped within the cavernous sinus. Bacteria are potent stimulators of thrombosis by mechanisms that include release of coagulative substances or toxins causing tissue damage, and the thrombus itself is an excellent growth medium for bacterial. Bacteria within the deeper layers of the thrombus are shielded from antibiotic penetration by the outer layers of the thrombus that they can subsequently infect. Etiology of CST In the early antibiotic era, paranasal sinusitis was responsible for up to 30 per cent of cases of septic CST.1 Although septic CST is now infrequently documented, contemporary reports suggest that a greater proportion of CST is due to sinusitis, and it is possible that this has become the most prevalent aetiology as a result of a disproportionately greater reduction in complicated facial infections. This may be explained by the fact that facial In sinogenic cases of septic CST, the sphenoid and/ or ethmoid sinuses are almost invariably involved. This contrasts to an indirect pathway that is more frequently responsible for the intracranial spread of ethmoid sinusitis, notably infection reaching the cavernous sinus via ophthalmic veins after breaching the lamina papyracea. Furthermore, as CT and MRI become more sensitive they may be able to confirm the diagnosis unequivocally at an earlier stage than could be achieved on clinical grounds alone. Traditionally, the most sensitive investigation of intracavernous occlusive defects has been venography, either via the orbital or inferior petrosal veins. Cerebral angiography had also been advocated as the primary confirmatory technique, since it frequently demonstrated abnormal findings such as narrowing or occlusion of the intracavernous segment of the internal carotid artery, and the venous phase images from angiography have shown poor filling of the cavernous sinuses or reversal of normal venous flow at the base of the skull. However, conventional venography and angiography have several drawbacks; they require iodinated contrast material that can cause dissemination of infection and thrombus extension in patients with septic thrombophlebitis, they are invasive and technically difficult in patients with periorbital inflammation and sepsis, and they are not always reliable (the venous phase images from technically well-performed angiograms demonstrate the cavernous sinuses in only 42 per cent of normal subjects). As a result of these limitations, these older techniques have a limited role in the modern diagnosis of septic CST. It is important to note that intracavernous filling defects may be observed using any imaging modality in pathologies other than thrombosis. These pathologies include thrombosed intracavernous carotid artery aneurysms, trigeminal neuromas, intracavernous metastases, and cartocio-cavernous fistulas. Using either CT or MRI, both direct and indirect signs of thrombosis are sought to increase the accuracy for diagnosing CST. Direct signs include expansion of the cavernous sinuses, convex bowing of the lateral wall, abnormal filling defects and asymmetry between each side. As the disease process becomes established, eye signs subsequently develop as a result of neural involvement, and this has been proven unequivocally by evidence of aberrant regeneration of the oculomotor nerve in one case that has survived. Typically, one eye is affected first and more severely in acute presentations of septic CST, and this is followed by signs in the other eye within 48 hours in most cases. Chemosis, periorbital oedema and proptosis are the most consistent (95 per cent) and visible features of septic CST. Impaired ocular motility invariably occurs and has different mechanisms; mechanical restriction caused by venous congestion of orbital tissues, extraocular muscle inflammation and infection, and inflammatory involvement of the oculomotor, trochlea, and abducent nerves within the orbit and cavernous sinus. Gaze paresis may develop sequentially, with lateral gaze being affected initially and most severely. This may be explained by the susceptibility of the abducent nerve to intracavernous pathology because of its intraluminal course. The trochlear nerve is affected least in septic CST, which may be explained by the fact that this nerve lies outside the muscle cone in the orbit, hence is spared from mechanical damage from muscle inflammation and orbital pressure.
